Our organization provides services of integrated model for the frail elders to put into “Care for elderly” and “Continuous Care” practice, which can enable them to receive professional nursing services and personal care without leaving their home and community and maintain maximum activity level; it can provide support for the elderly to enjoy a harmonious family life.

Service Scope

Rehabilitation Exercises

Rehabilitation Exercises as prescribed by Physiotherapist.

Personal Care Services

Personal care workers will assist the elders in bathing and care for incontinence.

Meals on Wheels Service

To deliver nutritious and delicious meals for the elders in need.

House Cleaning Services

To assist the elder people living alone or in need to clean their house.

Transportation and Escorting Services

Rehabus will assist the elder people and those on wheelchairs in medical follow-ups or other appointments.

Counseling Services

Social workers will provide the elders with training and groups to improve their mental health.

Home Safety Assessments and Modifications

Occupational therapists will evaluate and improve the residential environment for elders to avoid accidents at home.

Nurse visiting service

Nurses will visit to monitor health status and supervise on drug use.

Carer Support Services

To organize activities or talks to give elder carers guidance on caring skills.

Wound Care Services

Nurses or health workers will help the elders to assess and dress their wound.

Home Cognitive Training

To provide cognitive training to the elders and occupational therapists will design the training contents.

Escorting by Stair-climber Service

To escort the elders whose residence lacks elevator or lifter to enhance social integration.

Drug Management Service

Drugs are managed by professionals and dispensed to the elders to ensure good drup compliance.

Body Check Service

To organize annual health check for the elders.

Ophthalomological Examination

To organize ophthalomological examination for elders

Dietitian Service

To provide the elders with nutrition consulting service

Rehabus

There is a rehabus with 14 seats and lifting platform for wheelchair users.

Stair-climber

To assist the elders to climb stairs in order to create more opportunities for outdoor activities, then improving their life quality.

All kinds of rehabilitation supplies

The elders can borrow these rehabilitation supplies after assessed by professionals, such as wheel chair, shower chair, ripple bed and other walking aids. All kinds of cognitive training supplies can also be borrowed, such as tablet computers and customized card or borad games.
