Chu Lee Yuet Wah Day Care Centre for the Elderly - Introduction of Day Care Service

Service Aims

We adhere to the idea of "community care" and concept of "person-oriented" to provide medical service, nursing care and social activities to the elders who live in Sham Shui Po and Wong Tai Sin district in order to support the elders living in the community.

Service objective

To provide a series of supporting services which enable the elders to stay in a familiar community and enjoy their old age.

~     To provide various social, educational and developmental activities, encourage the elders make
good use of their leisure time, participating in the social affairs; develop personal potentials and construct a rich and colorful old age.

~     Assist the elders and the carers to explore and to make use of community resources.

~     Construct and encourage the community stakeholders to establish a caring community for the elderly in spirit of "respect" and "caring".

Facilities
Nurse station and Sickroom

There is a sickroom near the nurse station for the elders who feeling unwell can have a rest and close monitoring and caring by nurses.

Locker

Provide lockers for the elders to storage the personal belongings.

Sofa bed

The sofa beds are for the elders to have a nap or a rest.

Open physiotherapy activity area

There have the physiotherapy equipment, such as the standing machine “Easy stand”, the parallel bar, electronic pedal bikes, shoulder rotator and etc.

Simulation home area

Provide the personal care training to the elders and their carers.

Dementia area

~ This area using the Wander detector system and electronic door lock to protect the elder’s safety.

~ The Multi-sensory therapy room for sensory training for the dementia.

~ Provide diversify therapeutic training such as reality-orientation training, reminiscence and cognitive training for the elders.

Green lighting system

Enlarge the windows to make use of the natural lighting and increase the air convection.

Physiotherapy and Occupational therapy area

~ There has various kind of physiotherapy equipment for different ability elders rehabilitation

~ Provide cognitive training kits for the dementia in order to slow down the deterioration.

Laundry Room

There are advanced washing and drying equipment to provide laundry service to the service user.

~The elderly aged 60 or above who living in Sham Shui Po and Wong Tai Sin district

~They are required to go through the standardized care need assessment who are assessed to be of moderate or severe impairment level (exclude the respite service)

Application procedures

Formal members application

The elders can go to the nearest Integrated Family Service Centre (IFSC) or elderly services centers to apply and need to go through the standardized care need assessment.

Respite members application
Applicants or their families can call or visit the center to apply.

Full time member	Use the service 4 days or above per week	The applicant required to go through the standardized care need assessment that are assessed to be of moderate or severe impairment level of physical functions. And suitability for day care centers.	~Members are required to make a notice of withdrawal from service before14 days. ~If members leave the service due to hospitalization or other reasons more than 30 days,its services will be terminated. They require to re-apply the service again when they are in need.
Part time member	Use the service less than 4 days per week
Respite service	Provide short-term or temporary day care services for the frail elders	To apply the respite service can approach the center directly or apply by phone, fax and email	Withdraw service can make a notice by phone, fax, email or approach the center directly.
