Multi-disciplinary Outreaching Support Team for the Elderly (Tsuen Wan and Kwai Tsing)

Service Introduction

The Social Welfare Department (SWD) has launched a four-year Pilot Scheme on Multi-disciplinary Outreaching Support Teams for the Elderly (MOSTE) since mid-February 2019.  MOSTE, comprising social workers, physiotherapists, occupational therapists and speech therapists, provides outreach support services for the service users of more than 80+ private and contract residential care homes for the elderly (RCHEs) to address their social and rehabilitation needs. This Pilot Scheme has been regularized since February 2023.

Target group: Residents, carers and staff members of private, contract, self-financing RCHEs and Nursing Homes(NHs) in Tsuen Wan, Kwai Chung and Tsing Yi.


Service Scope

- On-site visits by social workers, occupational therapists, physiotherapists and speech therapists

- Provision of linkage activities to meet the social needs of the residents

- Clinical assessments, rehabilitation trainings and periodic reviews to the residents

- Consultations / training programmes to the staff members / carers of the residents of the RCHEs
