Objective

The Centre is established in 2016 and committed to provide comprehensive residential care service for the elderly.
A variety of social and recreational activities are provided to promote their physical and mental health, develop their
potential, and contribute their fruitful elder life. In addition, the Day Care Centre for the Elderly is also provides the
daytime support, nursing care, and social and recreational services, they services maintain the elders optimal level
of physical and mental health , enhance their social skills and thus, the services achieve the goal of " Ageing in Place"
 of order in Hong Kong.


Scop                                                                                                                                                                               
               

Dietary Service

Provide total 5 meals per day which include breakfast, lunch, dinner, afternoon tea, and night snack.

Nursing Care Service

24 hour nursing care duty which maintains as the all-rounded personal care service to the elder medical practitioner in-house services;

Personal Care Service

Provide shower, toilet, feeding, rehabilitation, laundry, and other personal care services.

Counselling and Referral Service

Provide emotional support and counselling service by registered social worker.

Physiotherapy Service

Arrange appropriate rehabilitation exercises to maintain the elder's health condition by physiotherapist.

Occupational Therapy Service

Provide different daily activity living skill training. A therapeutic groups or activities are designed by occupational therapist for the demented
elders, including of reality-oriented program, reminiscence program and sensory program.

Transportation Service

Transportation is provided to and from the day care centre.

Social and Recreational Activities

The social workers team organizes a variety of different types of group and festival celebrations to encourage the life-long learning,
maintain physical and mental health, and developing their personal interest and potential ability.

The Centre is to own around 1692 square meter with 2 floors of building.

Rest Room

These rooms are the place for their daily life and are equipped with electric hospital bed, pressure-relief mattress, air-conditioning,
heaters, wardrobe, lighting, fans, private bathroom etc.

Muti-activity Room (Activities, Canteen, Rehabilitation)

Ø   Centre organizes groups and activities regularly to make the elderly having fun.

Ø   Canteens are available on each floor of the Centre for the elderly.

Ø   rehabilitation equipment of different functions are available, such as ceiling hoist and bicycle, etc.

Ø   As quintessence in China, mahjong is one of the common social activities for the elderly. In addition, newspaper is available
for the elderly who is interested in reading and updates the new happened.

Reminiscence Corridor

Old fashioned items are displayed to enable them recall past and to stimulate the elderly memories by
reminiscence therapeutic program or activities

Resources Corner

Different categories of books are provided for the elderly and their families. In addition, Wi-fi and digital tab are provided
in
the home for the elderly to look for information by themselves.

Laundry

Washing machines and dryers are equipped to provide one-stop laundry services.

Kitchen

The kitchen can provide meal daily for approximately 120 elderly persons, including breakfast, lunch, 

afternoon tea and dinner.

Eligibility Criteria

Care and Attention Home with Continuum of care:
65 years old and above, to be assessed
of moderate impairment level under the Standardised Care Need Assessment Mechanism for Elderly Services: in poor health 

or suffering from functional disabilities to the extent that assistance in personal care, daily living activities and limited nursing

 care is necessary

Day Care Centre:
60 years old or above who is living in Hong Kong Island of South district: be assessed of moderate to severe impairment level under the 

Standardised Care Need Assessment Mechanism for Elderly Services: in poor health of suffering from functional disabilities to the extent

that assistance in personal care and daily living activities in daytime is necessary:

Fax the filled application form which will be gone through assessment period for 4 - 5 weeks for free.

Apply in the nearest Integrated Family Service Centres or Elderly Centres, and Standardized Care Need Assessment for Elderly Services of the Social Welfare Department will assess eligibility.
