Objective
Our organization provides service of integrated model for the weak and elderly to put into “Care for elderly” and ”Continuous Care” practice, which can enable them to accept nursing and care without leaving the familiar home and community environment and maintain maximum activity ability; it can provide support for elder carers to enjoy a harmonious family.

Procedures

Service users can
contact the nearby Integrated Family Service Center/ Integrated Service Center,
Medical Social Service Department, Community Center for the Elderly,
Neighborhood Center for the Elderly, Activity Center for the Elderly or
Rehabilitation Service Center, and then these service unities can assist to
provide referral services. Or they also apply to each integrated home care
service team directly. Any inquiry for the uniform assessment of elderly
service for weak case, please call Management Office of Uniform Assessment for
Elderly Service.
