Application for Non-Subsidized Residential Care Home Place:
Simply fax the completed application form to our home. After receiving the form, we will arrange an admission assessment, which takes approximately 4-5 weeks. The assessment is free of charge.

Application for Subsidized Residential Care Home Place:
Please apply at the nearest Integrated Family Service Centre or Elderly Services Centre. Eligibility for admission will be assessed by the Social Welfare Department's Standardized Care Needs Assessment Mechanism for Elderly Services.

Application for Day Care Centre Services:
Please apply at the nearest Integrated Family Service Centre or Elderly Services Centre. Eligibility will be assessed by the Social Welfare Department's Standardized Care Needs Assessment Mechanism for Elderly Services.

Temporary Day Care Centre Respite Service is also available. Applicants or family members may apply in person at our home.
