Background

With ageing of the Hong Kong population, an increasing need for long term care services is expected. In line with the Government’s policy direction and the benefits of developing a social infirmary service in the longer term, HA collaborates with a non-governmental organization (NGO), Po Leung Kuk, to enhance the choices of infirmary care services for applicants on the Central Infirmary Waiting List (CIWL) managed by HA. An Infirmary Service Public-Private Partnership Program (“Infirmary Service PPP” or the “Program”) is about to be implemented on a pilot basis, through contracting with Po Leung Kuk to operate infirmary services at the Wong Chuk Hang Hospital.

To ensure elderly under our care can live a healthy and enjoyable life by enabling them with different programs and activities, respecting their individual needs and looking after them with utmost care and concern.

• 全方位專業服務，包括中西醫、護士、物理治療師、職業治療師等，為院友設計個人照顧計劃及提供各項專業治療服務
• 護理員提供起居生活照顧及護理服務
• 社會工作員提供專業輔導及康樂服務
• 由專業護士及營養師設計健康餐單
• 護老者支援服務

評估程序

1. 經社區老人評估小組評估並已於中央輪候冊登記；及
2. 經社區老人評估小組再評估並確定健康狀況穩定，而沒有需要接受醫管局醫院療養服務持續醫療護理，且並無下列任何狀況：

- 病況需倚賴醫院儀器支援及專業護理；

- 發病率高或症狀難以控制而需於醫院接受護理；

- 雖然接受適當的醫療護理，但仍經常入住醫院；

- 需要在醫院接受複雜的護理照顧。

溫馨的家居環境

提供溫馨的「小家舍」環境，強化「家」的感覺。

多用途活動室

備有電視及舒適的座椅，提供休閒活動空間給院友歡聚。

康復治療室

由專業物理治療師和職業治療師教導院友使用不同健體及復康器材，增強活動能力。

感觀治療室

為認知障礙症長者提供感觀訓練。

健康資源閣

讓家屬獲得最新的健康資訊。設有懷舊物資，透過懷緬治療，讓長者懷緬昔日美好時光。

懷舊閣

放置各種懷舊物品和擺設，讓院友懷緬昔日生活點滴。
